Fill Form with Web Browser

Hi,

I made a LC Form to be filled. It work properly,

But When i publish it to my WordPress site and i try to fill it in Chrome or Firefox Browser, it don't save the fill form.

And when i doenload the form, the ability to fill doesn't work anymore ?

Any idea ??
