data connection: sample XML data

Ialreadypostasimilaritembefore, buttherewas noanswer, Isaid that I didnot quiteexplain the problem.
I have a form,in the openningthedata inan XML documentmust be added(name,address, postalcode,phone number)
Howshould I do?
I triedthe optiondata connection,it does not work,the information does notappearin the openning of the form.
